ComForCare Home Health Care - Clarksville in Hendersonville, TN is seeking a compassionate Care Coordinator to join our team. You will serve as the vital link between clients, families and caregivers, ensuring high-quality in-home care with a 50% office and 50% field schedule.

Responsibilities include coordinating caregiver schedules, assessing client needs, developing care plans with the care team, monitoring performance and maintaining records.

Qualifications

• Prior experience in home care, healthcare coordination, or related field preferred.
• Strong organizational and multitasking skills in a fast-paced environment.
• Excellent communication and interpersonal skills with clients, families, and staff.
• Proficiency with scheduling software and basic computer applications.
• Ability to problem-solve quickly and remain calm under pressure.
• Compassionate, patient-centered mindset with a commitment to quality care.
• High school diploma or equivalent required; associate's or bachelor's degree in a health-related field a plus.

In this role, you'll be the vital link between clients, caregivers, and families — ensuring every individual receives the highest quality of in-home care. Must be willing to work part-time care coordinator 50% in the office and as a caregiver 50% in the field with our Clients.
