Job summary

Heritage Behavioral Health Center in Decatur is seeking a Care Coordinator for its outpatient mental health and substance use teams. This full-time role runs Monday–Friday, 8 AM–5 PM with every other Friday off (paid Wellness Day). You will help clients navigate care, coordinate with primary care and community partners, and document progress in the EHR.

Strong communication, empathy, and teamwork are essential to support diverse clients and improve outcomes.

Qualifications

High School Diploma/GED plus 5 years of supervised mental health or social service experience, OR a Bachelor's degree in social work, psychology, education, or a related human services field
Minimum age of 21
Valid driver's license, reliable transportation, and proof of auto insurance
Strong communication skills and the ability to build rapport with diverse clients
Comfort with electronic health records (EHR) and Microsoft Office
A collaborative, adaptable, and empathetic approach to client care

Responsibilities

Ensure clients get connected to behavioral health treatment, primary care, specialty care, and community resources
Follow up proactively with clients to track progress and troubleshoot barriers to care
Coordinate with primary care teams, behavioral health providers, and community partners on appointments and treatment plans
Educate clients and families on care continuity and available resources
Document client progress and service engagement in electronic health records
Help clients navigate healthcare and social service systems
Participate in interdisciplinary team meetings and ongoing trainings
