Job summary

Sierra Health and Wellness is hiring a Care Coordinator - SUD Counselor to support clients in treatment for chemical and alcohol dependence. You will assess needs, develop treatment plans, and coordinate referrals for mental health or medical services.

The role includes crisis management, leading group therapies, documenting progress, and coordinating discharge and family involvement. A high school diploma, D&A counselor certification, and at least one year of experience are required.

Qualifications

• Must have High School diploma or GED.
• Certification as Drug and Alcohol Counselor required.
• At least one year of experience in Drug and Alcohol counseling.
• Familiar with HIPAA and client confidentiality.
• Valid driver’s license and insurability.

Responsibilities

• Provide assessment and participate in treatment planning with clients.
• Review client charts for paperwork and signatures; weekly audits.
• Facilitate discharge planning and community outreach.
• Respond to emergencies and document noteworthy events.
• Meet with clients individually at least twice weekly and maintain notes.
• Attend staff meetings and trainings.

Generally, your job is to assess your clients' immediate and long-term needs, and develop a treatment plan to reduce or eliminate any substance abuse or criminal thinking. Your job may consist of making referral appointments to meet mental health or medical needs; work with the Job developer to build your client’s employability or start the process for SSI depending on your clients’ needs and capabilities. The SUD Counselor is responsible for documenting client behavior in group participation and counseling. Counselors will also be providing crisis management and counseling services while also running group therapies and education for the clients and family/significant others in a therapeutic setting.
