Job summary

Centerstone is a leading nonprofit behavioral health system seeking a Care Coordinator to provide wraparound and intensive care coordination for youth and families with high needs. The role travels within Madison, Calhoun, Jersey, Greene, Macoupin, Montgomery, and Bond counties and emphasizes collaboration with families and providers to implement growth plans.

Qualifications

  • Bachelor’s Degree in a human services‑related field or equivalent direct mental health experience.
  • Associate’s Degree in human services plus at least three years of direct mental health experience.
  • High School diploma or equivalent with a minimum of five years of supervised behavioral health experience.

Responsibilities

  • Provide Care Coordination for High Fidelity Wraparound (Tier 1) or Intensive Care Coordination (Tier 2).
  • Collaborate with clients, families, and other providers to implement plans.
  • Deliver outreach, advocacy, support and follow‑up to assigned clients.
  • Document outreach attempts and ensure services in the personal growth plan are delivered as scheduled.
  • Offer ongoing case management: assessment, planning, linking, monitoring, referral and advocacy.
  • Provide case management to Family Support Program (FSP) and/or Specialized Family Support Program (SFSP).
  • Help clients build a natural support team and integrate community supports into growth planning.
  • Present information to ensure quality of care and plan service changes.
  • Identify and report all Critical Incidents as required by policy.
  • Provide community education as assigned.
  • Conduct in‑person community team meetings and review IMCANS.
  • Support crisis services and evaluate prevention and safety plans.
  • Collaborate with other service teams to meet needs of Pathways clients.

SUMMARY OF POSITION

Travel within the following counties: Madison, Calhoun, Jersey, Greene, Macoupin, Montgomery, and Bond. The role provides wrap‑around and/or intensive care coordination to youth and families with high needs.

ESSENTIAL DUTIES
  • Provide Care Coordination and support for High Fidelity Wraparound (Tier 1 clients) or Intensive Care Coordination (Tier 2 clients).
  • Work in consultation and collaboration with clients, family members and other providers to implement plans.
  • Deliver assertive outreach, advocacy, support and follow‑up to assigned clients.
  • Document all outreach attempts and ensure services outlined in the personal growth plan are delivered in the identified frequency.
  • Offer ongoing case management services, including assessment, planning, linking, monitoring, referral and advocacy.
  • Provide case management to Family Support Program (FSP) and/or Specialized Family Support Program (SFSP) youth.
  • Assist clients in building a natural support team and integrate community supports into the personal growth planning process.
  • Present relevant information to assure quality of care, gather feedback, and plan changes in service provision.
  • Identify and report all Critical Incidents as required by Centerstone policy.
  • Provide community education as assigned.
  • Conduct in‑person, community‑based child and family team meetings and review IMCANS.
  • Support crisis services and evaluate prevention and safety plans.
  • Collaborate with other service teams to meet the needs of clients assigned through Pathways, including connecting clients to appropriate services and advocating on their behalf.
KNOWLEDGE, SKILLS, AND ABILITIES
  • Effectively communicate via written, verbal, in‑person and virtual methods.
  • Demonstrate customer service/experience techniques.
  • Understand the role and function of various community resources.
  • Apply problem‑solving skills.
  • Know basic dynamics of service recipients within assigned disability groups.
QUALIFICATIONS
  • Bachelor’s Degree in a human services‑related field, OR Associate’s Degree in human services plus at least three years of direct mental health experience, OR High School diploma or equivalent with a minimum of five years of supervised behavioral health experience (subject to approval).
  • Prior experience as detailed above.
CERTIFICATION / LICENSURE
  • None required.
TIME TYPE AND PAY
  • Full‑time position.
  • Pay Range: $16.50 – $23.10 per hour.
