Qualifications

Job description

Immediate need for a talented Care Coordinator. This is a 3+ months contract opportunity with long-term potential, located remotely in TX (CST timezone). Please review the job description below and contact me ASAP if interested.

Pay Range: $18 - $22/hour. Employee benefits include health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:
  1. Support care management activities and teams to ensure services are delivered effectively, maintaining continuity of care and member satisfaction.
  2. Perform member outreach via phone or home visits, documenting care plans and activities.
  3. Participate in a family team environment that promotes a positive and engaging atmosphere.
  4. Make outbound calls to members for Health Needs Assessments and handle inbound calls for the same purpose.
  5. Achieve a screening rate of 35% as per the State Contract.
  6. Maintain high energy, demonstrate call center experience, be a self-starter, reliable, and possess strong communication skills.
  7. Work hours are 8 am to 5 pm CST with a one-hour lunch break.
  8. Manage call volume and productivity, with monthly call and HNA audits.
Key Requirements and Technology Experience:
  1. Experience in phone-based customer service, basic computer skills (MS Office: Excel, Word, Outlook), and 2-3 years in managed care or physician's office settings.
  2. High School Diploma or equivalent required.
  3. Experience working directly with healthcare providers or patients, including scheduling.
  4. Handle 35-40 outbound calls daily, with a call audit score of 90% or higher and HNA audit score of 90% or higher.
