Harbor is seeking a Care Coordinator responsible for comprehensive, multi-system care coordination activities for youth with complex needs and their families.
Position is full-time, 40 hours per week. Candidates preferably reside in Seneca County. The role is remote and community-based.
Education, Experience, and Other Requirements
- At least 3 years of experience (or 2 years with an Associate or Bachelor degree; or 1 year with a Master degree) in providing community-based services to children, youth, and their families or caregivers, in areas such as behavioral health, child welfare, disabilities, juvenile justice, or related fields.
- Background in family systems, community resources, case management, counseling, child protection, or child development.
- Culturally competent with training and experience to manage complex cases.
- Qualifications to work with children and families experiencing SED, trauma, co-occurring disorders, engaged with multiple child-service systems.
- Complete high-fidelity wraparound training from an ODM-recognized entity (COE).
- Effective verbal and written communication skills. Willing to represent the organization within the healthcare community.
- Proficient in computer use.
- Valid Driver’s license, acceptable driving record, and insurability through Harbor’s insurance.
- CPR/First Aid and NCI/CPI certifications required within 90 days of employment.
- Preference for residents of the community served.
- Minimum Internet Service: 5 Mbps upload, 10 Mbps download via cable or fiber. No DSL, satellite, or cellular. Business class preferred. Ability to connect via Ethernet cable.
Essential Job Competencies and Primary Duties
- Engage children/youth with behavioral health needs and their families/caregivers (caseload: 1:10 for ICC, 1:25 for MCC).
- Maintain knowledge of community resources; link youth and families to supports.
- Conduct comprehensive CANS assessments and develop strength-based, child and family-centered care plans.
- Monitor and update plans, addressing challenges and barriers.
- Facilitate Child and Family Team meetings, ensuring the child's and family's voice and choices.
- Encourage natural supports and community integration.
- Facilitate access to clinical and psychiatric consultations.
- Develop and review safety and crisis plans with youth and families.
- Complete required documentation and engagement activities per CME rules.
- Work flexible hours, including evenings and weekends, as needed.
- Provide services in home and community settings.
