Orlando Health Medical Group seeks a Care Transition Navigator to coordinate patient care transitions within the health system. You will work with clinical and administrative stakeholders to ensure resources are in place for timely, safe, and effective discharge planning.

The role requires knowledge of medical terminology, strong communication, and the ability to develop post-acute care plans. You will collaborate across sites to support value-based care initiatives.

Qualifikationen

• High school diploma or GED required.
• One (1) year experience as a Medical Assistant, Paramedic, Emergency Medical Technician, Military Allied Health Professional, Nursing Assistant or related health care role in value-based care or physician offices required.
• None.

Aufgaben

• Possesses a strong understanding of medical terminology and healthcare operations.
• Takes initiative to develop knowledge to perform at a high level in the care transition navigation role.
• Ensures compliance with risk management programs and corporate quality initiatives.
• Partners with healthcare entities and physician practices to foster integrated relationships with patients and families.
• Assists patients and caregivers in navigating care services post-hospitalization and develops post-acute care plans.
• Engages with patients using strong communication skills and leverages patient feedback to identify service needs.
• Distributes educational materials to patients to address social determinants of health and prevent readmissions.
• Advocates for patient needs by ensuring access to prescriptions and care services.
• Collaborates with population health and value-based care teams to adjust care plans as needed.
• Maintains proficiency with organizational information systems to support care transitions.
• Performs other duties to support the population health team and adheres to policies.
• Maintains punctual attendance and compliance with policies.
