Qualifications

• RN Diploma or Nursing Degree with 2+ years in community-based services.
• Bachelor's degree in Social Work with 1-2+ years in community-based services.
• 5+ years of clinical experience.
• 1-2+ years supervisory experience of professional staff.

Responsibilities

• Oversee two teams of Administrative Support Specialists providing Care Coordination referral and admin support.
• Lead care coordination services for State Public Programs and Medica members; partner with Clinical Care Manager to implement improvements.
