Job summary

Kandiyohi County is seeking a Care Coordination Supervisor to lead the county's Care Coordination team and oversee care coordination programs for MSHO, MSC+, and SNBC. The role ensures quality, timely care plans, and compliance with DHS, CMS, and NCQA requirements.

The supervisor will drive staff training, performance management, and collaboration with multiple partners to optimize care delivery and program outcomes for county residents.

Qualifications

• Bachelor's degree in Public Health, Nursing, Social Work, or related field.
• Minimum of three years of supervisory experience in human services/public health.
• Extensive knowledge of case management and care coordination.
• Experience with MSHO, MSC+ and SNBC programs.

Responsibilities

• Lead the Care Coordination team and supervise staff to ensure quality care.
• Provide fiscal oversight and manage the care coordination budget.
• Ensure compliance with DHS, CMS, NCQA and county delegation agreements.
• Collaborate with MCO care management, providers, and multidisciplinary teams to coordinate member care plans.
• Monitor care plans, health risk assessments, and MOC documentation.
• Participate in staff development, training, and performance evaluations.
• Represent the agency in professional organizations and community initiatives.
