Qualifications

• RN diploma or nursing degree and at least 2 years of community-based services experience.
• Bachelor's degree in Social Work with 1–2+ years of community-based services.
• Minimum 5 years of clinical experience and 1–2 years of supervisory experience.

Responsibilities

• Provide direct supervision to Care Coordinators and mentor team.
• Educate team on using membership information and coordinating interventions for vulnerable populations.
• Manage care coordination for assigned members and coordinate with physicians and other providers.
• Participate in on-call program as needed.
