Job summary

Sentara Virginia Beach General Hospital in Virginia Beach, VA is seeking a Supervisor, Care Coordination to lead daily operations, coordinate work, quality and service, and provide clinical oversight to the Managed Care Coordination team.

The role requires a Master’s degree in Counseling, Social Work or Psychology, LCSW license, 3 years case management, 5 years acute care, and 1 year working with vulnerable populations; prior supervisory experience is preferred.

Qualifications

• Master's level degree in Counseling, Social Work or Psychology is required.
• 3 years of case management experience is required.
• 5 years of acute care experience is required.
• 1 year of experience working with vulnerable populations is required.

Responsibilities

• Assumes responsibility and leadership for daily operations including coordination of work, quality and service.
• Provides leadership in ongoing case manager competency assessment, needs identification and educational offerings.
• Provides educational services and clinical oversight to the Managed Care Coordination team.
• Participates in the work activities of assigned teams and provides care coordination services as needed.
