Job summary

University Health is seeking an experienced Social Work Manager to lead a dedicated team, ensuring a smooth patient flow from preadmission to discharge and follow-up in line with patient and payer requirements.

You will collaborate with a multidisciplinary team to create patient-centered treatment plans, facilitate service delivery, evaluate effectiveness, and track outcomes, serving as the manager of the social work team.

Qualifications

• Master’s degree in social work required.
• Minimum 1 year recent management or senior social worker experience.
• 3 years hospital (inpatient) experience.
• 2 years inpatient case management or utilization management preferred.

Responsibilities

EDUCATION/EXPERIENCE

A master’s degree in social work is required. A minimum of one year recent management or senior social worker is required with 3 years of hospital (inpatient) experience. 2 years of inpatient case management, or utilization management is preferred.

LICENSURE/ CERTIFICATIONS

Certification in Case Management or Care Coordination and Transition Management is highly desirable (CCM or ANCC).
