Qualifikationen

• Master’s degree in counseling, social work or psychology (required).
• Licensed Clinical Social Worker license (required).
• LMFT (preferred).
• LPC (preferred).
• 3 years case management experience (required).
• 5 years acute care experience (required).
• 1 year working with vulnerable populations (required).
• Prior supervisory experience (preferred).

Aufgaben

• Assumes responsibility and leadership for daily operations including coordination of work, quality, and service.
• Facilitates the work of assigned team members.
• Provides leadership in ongoing case manager competency assessment, needs identification and educational offerings.
• Provides educational services and clinical oversight to the Managed Care Coordination team.
• Participates in the work activities of assigned teams and provides care coordination services as needed.
