Job summary

Northwell Health seeks a Care Management professional to coordinate care for families and caregivers, using patient-centered approaches to improve health outcomes. You will engage patients, help build trust, and support health goals.

Responsibilities include liaising with hospitals and primary care teams, managing care panels to reduce gaps, and developing individualized plans for high-risk patients while outreaching to communities to improve access to services.

Qualifications

• High School Diploma or equivalent required.
• Bachelor's or Master's degree strongly preferred.
• 1–3 years of relevant experience required.
• Health home or care management experience preferred.
• NYS Driver's License preferred.
• Bilingual Spanish preferred.

Responsibilities

• Uses patient-centered motivational interviewing techniques to build rapport and help patients improve their health.
• Supports the primary care team by providing panel management to decrease patients lost to care and to address non-compliance; develops and adjusts care plans for high-risk patients addressing medical and social barriers.
• Acts as a professional liaison between hospitals, primary care providers, specialists, and community resources to ensure patient-centered care coordination.
• Identifies and tracks high-risk populations and others due for preventive or chronic care services.
• Identifies and tracks patients discharged from inpatient services or the emergency department.
• Uses team-based communication to close referral loops; performs outreach in primary care sites, homes, hospitals, and neighborhoods.
