ncgCARE is seeking a Care Management Extender in Lumberton, NC to support Tailored Care Management efforts, linking members to medication management, coordinating physical and behavioral health care needs, and facilitating community resources. Qualifications

• High school diploma or equivalent.
• Must be 18 years or older.
• Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist.
• A parent or guardian of an individual with a behavioral health condition and has at least two years of direct experience navigating the Medicaid delivery system on behalf of that individual.
• Have two years of paid experience performing the functions described in the Extender Functions section, with at least one year serving the Tailored Care Management eligible population.
• Upon hire, completion of general onboarding training; within 60 days completion of NC DHHS Tailored Care Management training.
• No driving infractions in the last 5 years.

Responsibilities

• Conduct regular outreach to members by phone and in person to assess needs, provide support, and monitor progress.
• Connect members and families to healthcare services, housing assistance, food resources, and community programs.
• Assist in developing and updating individualized care plans to support members' health and wellness goals.
• Coordinate communication among healthcare providers, members, and families to ensure continuity of care and follow-up on referrals.
