Job summary

MSCCN is seeking a Care Coordinator to support Portales/Clovis, NM areas. The role involves coordinating care for individuals, building care plans, and ensuring quality outcomes through collaboration with clinicians and community resources.

Responsibilities include needs assessment, risk evaluation, and advocating for member-centered services, both virtually and in-person as required by contract.

Qualifications

• 3-5 years' experience in Social Work, Nursing, or Healthcare field.
• Experience in utilization management, quality assurance, home or facility care, community health or long-term care.
• Ability to analyze trends from decision support systems and make data-driven decisions.

Responsibilities

• Coordinate care for individuals using assessment, care planning, implementations and monitoring.
• Promote efficient use of clinical and financial resources to improve outcomes and member satisfaction.
• Serve as point of contact to ensure services are rendered appropriately across transitions.
• Educate providers and families on care coordination strategies with a member-focused approach.
• Collaborate with the interdisciplinary care plan team to address member needs and disease processes.
