Job summary

Ennoble Care seeks a full-time Patient Care Coordinator in Cincinnati, OH to support high-quality home-based care. You will work with clinicians, staff and families to manage care plans, scheduling, and communication, ensuring compassionate, organized support for patients with chronic conditions.

The role requires on-site presence, strong phone etiquette, and proficiency with Microsoft Office and EMR systems.

Qualifications

• Experience in healthcare or care coordination is preferred.
• Ability to document accurately and maintain patient records.
• Comfortable communicating with patients, families and clinicians.

Responsibilities

• Create and manage individualized patient care plans and coordinate services using the EMR.
• Maintain frequent contact with patients to support care coordination and program compliance.
• Document all communications accurately.
• Coordinate with Primary Care Clinician on patient needs and care plans.
• Inform Team Supervisor of issues affecting the care plan.
• Collaborate with cross-functional teams to meet patient needs.
• Develop proficiency with technology including secure email and EMR.
• Adhere to documentation protocols and daily work logs.
• Deliver excellent customer service evidenced by patient feedback.
