Job summary

Qualifications

• Masters in Social Work required.
• AZ MSW licensure (MSW:AZ) expected.
• 1 year post-MSW clinical or medical experience required.

Responsibilities

• Perform social work assessments and interventions for hospitalized and ED patients.
• Conduct psychosocial assessments and crisis intervention.
• Coordinate referrals for financial or other resource needs and arrange family/patient meetings with the care team.
• Arrange post-acute placement on complex discharges and engage community resources.
