Qualifikationen

• Requires LMSW license in Arizona (AZ).
• MSW from CSWE-accredited program.
• Minimum 1 year post-MSW clinical or medical setting.

• Ability to manage a fast-paced environment with changing priorities.
• Collaborates with an interdisciplinary care team.

Aufgaben

• Perform comprehensive psychosocial assessments and develop treatment plans addressing medical, social, and resource needs.
• Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients and families.
• Serve on hospital committees and present in-services to staff.
• Participate in performance improvement programs as needed.
