Geisinger Wyoming Valley is seeking an experienced RN to join the Care Coordination team. The role emphasizes patient-centered planning, collaboration with the health care team, and ongoing assessment to optimize outcomes.

The position requires at least three years of RN experience and nursing education including Specialty Training and a Bachelor’s degree preferred. Benefits begin on day one, with strong PTO and tuition support options.

Competenze

• RN license and current clinical practice readiness are required.
• Three or more years of RN work experience is required.
• Education: Specialty Nursing training; Bachelor’s in Nursing preferred.

Mansioni

• Assess, plan, implement, coordinate, monitor and evaluate options to optimize patient health status.
• Integrate evidence-based guidelines into treatment plans for patient-centered care.
• Develop care systems to monitor progress and promote early intervention.
• Collaborate with patient, family, primary care provider and health care team to coordinate services.
• Maintain documentation for all case management activities and analyze data to adjust plans.
