Geisinger is seeking an RN Hospital Case Manager to guide patients through their care journey, coordinating services and resources while partnering with the care team to support optimal outcomes.

The role emphasizes assessment, planning, implementation, and coordination of care with a focus on evidence-based guidelines and patient-centric plans. The position requires strong collaboration with families and healthcare providers in a hospital setting.

Competenze

• At least three (3) years of RN work experience required.
• Full-time, exempt schedule with Monday–Friday days and weekend rotation.

Mansioni

• Assesses, plans, implements, coordinates, monitors and evaluates options and services to optimize patient health status.
• Integrates evidence-based guidelines and protocols to develop patient-centric treatment plans.
• Develops care systems that monitor progress and promote early intervention in acute situations.
