Role

The Care Coordinator RN (CC RN) assesses transitional care needs, coordinates care across the continuum, and engages with patients and families to ensure care goals are met.

Responsibilities

Conduct psychosocial and functional assessments, develop individualized discharge plans, facilitate patient/family education, and collaborate with interdisciplinary teams including physicians and social workers. Manage discharge processes, identify and resolve barriers to timely discharge, and ensure proper documentation of care and interventions. Participate in interdisciplinary rounds and post-acute referrals to optimize patient outcomes. Work proactively to facilitate care progression, address delays, and promote resource utilization.

Qualifications & Skills

Requires a nursing license (RN) with at least 1 year of acute care experience. Must have excellent communication, assessment, organizational, and problem-solving skills. Knowledge of regulations, community resources, and care coordination processes is essential. Ability to work independently, handle stress, and collaborate effectively with healthcare teams and patients.

Additional Requirements

Completion of mandatory education and certifications such as BLS. The role involves supporting department goals, mentoring, and adhering to organizational policies.
