Job summary

Tuba City Regional Health Care Corporation (TCRHCC) seeks a Referral Coordinator / Quality Improvement Specialist for its Community Health Center. The role coordinates care for patients requiring further evaluation and advocates for options and services to meet health needs through available resources.

This grant-funded position emphasizes collecting and reporting program quality measures and outcomes, collaborating with CHC teams, and improving care coordination and patient access.

Qualifications

• Education: High School diploma or GED.
• Experience: 1 year in care coordination or similar health care field.
• Certification: Current Basic Life Support (BLS) per AHA; must maintain.
• Other skills: strong communication, reliability, teamwork, and computer literacy.

Responsibilities

• Ensure referrals are addressed in a timely manner and prioritized.
• Assist in development of performance improvement measures for accreditation.
• Lead multidisciplinary team to collect and submit required reports.
• Collaborate with Case Management, Utilization Review, PHN, and other teams.
• Maintain resource database for timely patient referrals.
• Monitor referrals to ensure continuity of care and follow-up.
• Develop reports and support policy and program objectives.
• Provide information to patients and families for informed decisions.
• Document referrals and track outcomes accurately.
• Identify community resources and establish service provider relationships.
• Report performance and quality improvement progress to leadership.
• Enhance CHC’s PI/QI capacity to meet public health goals.
• Enter electronic health records data pertinent to referrals.
