Job summary

Saint Joseph’s Hospital & Medical Center in Phoenix, AZ, is seeking a Care Coordination Nurse to lead discharge planning for inpatient care from birth to adult. You will coordinate clinical and social services, advocate for patients, and balance needs with cost-effective resources.

You will conduct and update discharge assessments, facilitate multidisciplinary plans, and oversee implementation while promoting timely transitions to post-acute care and reducing readmission risk.

Qualifications

• Two years acute hospital clinical experience or a Masters degree in Case Management or Nursing field in lieu of one year experience
• Graduate of an accredited school of nursing
• Registered Nurse: AZ or Compact license
• American Heart Association Basic Life Support (BLS), within 90 days
• Knowledge of managed care and payer environment
• Must have critical thinking and problem-solving skills
• Collaborate effectively with multiple stakeholders
• Understand how utilization management and case management programs integrate
• Ability to work as a team player and assist other members of the team where needed
• Knowledge of CMS standards and requirements

Responsibilities

• Completes and documents a discharge planning assessment on patients identified by the designated screening process, reassess and update the plan as needed
• Facilitates development of a multidisciplinary discharge plan with health team members, patient and post-acute providers
• Oversees and evaluates the implementation of the discharge plan
• Collaborates with the multidisciplinary team to ensure progression of care and appropriate utilization of inpatient resources using evidence-based guidelines
• Collaborates with the healthcare team and post-acute service providers to ensure timely transitions to appropriate post-acute services based on clinical needs
• Dignity Health Education Benefit program for benefit-eligible employees after 180 days (up to $18,000 over five years)
