Position: Care Coordination Nurse (Remote, Augusta, ME)

This role requires an active nursing license in Maine. The position can be performed remotely.

Required Qualifications

• Licensed as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in Maine, issued by the State Board of Nursing. RN preferred; LPN acceptable if RN is unavailable.

Job Responsibilities

• Develop and implement care plans to reduce avoidable emergency department visits for MaineCare members.
• Collaborate with hospitals and providers to educate and coordinate care.
• Monitor work queues and complete assigned tasks within the care team.
• Collect data, prepare reports, and evaluate wellness programs.
• Create care plans for complex MaineCare members.
• Participate in community wellness initiatives and outreach via phone, mail, and fax.
• Attend community meetings and perform additional duties as assigned.

Knowledge, Skills, and Certifications

• Understanding of professional nursing principles and standards of care.
• Ability to plan, organize, and communicate effectively.
• Knowledge of community resources, regulations, and Maine Medical Assistance Manual.
• Critical thinking, decision-making, and relationship-building skills.

Minimum Qualifications

• Three years of progressive nursing or healthcare experience.
• Active Maine RN or LPN license; RN preferred.

This position does not sponsor work authorization or STEM EAD extensions. Compensation varies based on location, experience, and other factors, with a typical range up to $35.18/hour. Benefits are provided.
