Saint Joseph’s Hospital and Medical Center in Phoenix, a 571-bed nonprofit founded by the Sisters of Mercy, seeks a Care Coordination Nurse to lead discharge planning for our diverse inpatient population from birth to adult. You will orchestrate healthcare delivery and social services, balancing patient needs with cost-effective resources.

You will guide multidisciplinary teams, oversee discharge plan implementation, reduce readmission risks, and educate patients and families.

Qualifications

• Two (2) years of acute hospital clinical experience or a Masters degree in Case Management or Nursing field in lieu of one (1) year experience.
• Graduate of an accredited school of nursing.
• Registered Nurse: AZ or Compact license.
• American Heart Association Basic Life Support (BLS), within 90 days.
• Knowledge of managed care and payer environment.
• Must have critical thinking and problem-solving skills.
• Collaborate effectively with multiple stakeholders.
• Understand how utilization management and case management programs integrate.

Responsabilités

• Completes and documents a discharge planning assessment on patients identified by the screening process or upon request.
• Facilitates the development of a multidisciplinary discharge plan engaging the health team, patient, and post-acute providers.
• Oversees and evaluates the implementation of the discharge plan.
• Collaborates with the multidisciplinary team to ensure progression of care and appropriate utilization of inpatient resources.
• Collaborates with post-acute service providers to ensure timely transitions to appropriate post-acute settings.

Bachelor's degree in Nursing (BSN)
