Job summary

Jobtailor is seeking a Registered Nurse to perform comprehensive assessments for new admissions at acute care facilities near Charlotte, NC. The role emphasizes discharge planning and care coordination across hospital settings, with documentation in EMR systems and adherence to PHI/HIPAA standards.

The position requires 3+ years of acute care experience, NC RN license, valid driver’s license, TB screening, and a private home workspace with reliable internet. Onsite work Monday–Friday, 8am–5pm.

Qualifications

• Active RN license in North Carolina.
• Minimum 3 years acute care nursing experience.
• Experience with discharge planning or care coordination.
• Experience with electronic medical records.
• Valid driver’s license and dependable automobile.
• Proof of personal vehicle liability insurance.
• TB screening participation.
• ACHA Level II background clearance, including fingerprinting.
• Other credentialing as required by the facility.
• Private workspace for remote documentation.
• Home internet with adequate speeds (25 Mbps down / 10 Mbps up).
• Dedicated workspace without interruptions to protect PHI/HIPAA information.

Responsibilities

• Conduct comprehensive face-to-face assessments of newly admitted members at assigned acute care facilities.
• Evaluate members’ physical, psychosocial, and health needs and refer to Humana programs as appropriate.
• Support and educate members and families on discharge planning from admission through post-discharge.
• Facilitate follow-up visits with primary care providers.
• Identify gaps in care, safety concerns, and barriers to discharge.
• Develop and implement strategies to address care and discharge barriers.
• Collaborate with attending providers, facility case managers, and other healthcare professionals.
• Coordinate timely care and effective discharge planning.
• Educate members on diagnoses, medications, and preventive measures.
• Complete required documentation accurately and promptly.
• Maintain infection control, safety, and patient privacy standards.
• Work onsite at assigned facilities with limited remote documentation and coordination hours.
• Work Monday through Friday, 8:00 AM-5:00 PM local time.

Requirements

• Current, unrestricted Registered Nurse (RN) license in the state of North Carolina
• Must live within a 30-60 minute driving distance of Atrium Health Pineville, 10628 Park Road, Charlotte, NC
• Minimum of 3 years recent acute care nursing experience
• Experience with discharge planning or care coordination in a hospital or health plan setting
• Experience with Microsoft Office applications: Word, Outlook, and Excel
• Experience with electronic medical records
• Valid driver's license
• Dependable automobile
• Proof of personal vehicle liability insurance
• Participation in TB screening
• ACHA Level II background clearance, including fingerprinting
• Other credentialing and screening as required by the assigned facility
• Private workspace for remote documentation
• Home internet with minimum 25 Mbps download and 10 Mbps upload speeds
• Dedicated workspace without ongoing interruptions to protect PHI/HIPAA information
