Job summary

Atrium Health Cabarrus is seeking a Registered Nurse to assess, coordinate discharge planning, and educate patients in an acute care setting. The role involves direct patient assessments, referrals to Humana programs, and collaboration with care teams to ensure smooth transitions from admission to post-discharge.

Ideal candidates will have at least 3 years of acute care experience, NC RN licensure, and proficiency with MS Office and EMR systems.

Qualifications

• Current, unrestricted RN license in North Carolina.
• Within 30–60 minute driving distance of Atrium Health Cabarrus, Concord, NC.
• Minimum 3 years recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital/health plan setting.
• Experience with MS Office and electronic medical records.
• Valid driver’s license and personal vehicle liability insurance.
• TB screening and ACHA Level II background clearance (fingerprinting).
• Other facility credentialing as required.
• Private workspace for remote documentation and home internet with adequate speed.

• Support and educate members and families on discharge planning from admission through post-discharge.
• Facilitate follow-up visits with primary care providers.
• Identify gaps in care, safety concerns, and barriers to discharge.
• Develop and implement strategies to address care and discharge barriers.
• Collaborate with attending providers and facility case managers.
• Provide education on diagnoses, medications, and preventive measures.
• Complete required documentation accurately and promptly.
• Maintain infection control, safety, and patient privacy standards.
• Work onsite at Atrium Health Cabarrus with limited remote documentation hours.

Requirements

• Current, unrestricted Registered Nurse (RN) license in the state of North Carolina
• Must be located within a 30–60 minute driving distance of Atrium Health Cabarrus, 920 Church Street North, Concord, NC
• Minimum of 3 years recent acute care nursing experience
• Experience with discharge planning or care coordination in a hospital or health plan setting
• Experience with Microsoft Office applications (Word, Outlook, Excel) and electronic medical records
• Valid driver’s license, dependable automobile, and proof of personal vehicle liability insurance
• Participation in Tuberculosis (TB) screening and ACHA Level II background clearance, including fingerprinting
• Other credentialing and screening as required by the assigned facility
• Private workspace for remote documentation
• Home internet with minimum download speed of 25 Mbps and upload speed of 10 Mbps
• Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
