Job summary

Jobtailor is seeking a dedicated Registered Nurse to conduct comprehensive face-to-face assessments of newly admitted members in nearby acute care facilities and coordinate discharge planning from admission through post-discharge.

You will collaborate with attending providers, case managers, and health professionals to address care gaps, educate members and families, and ensure accurate documentation while upholding infection control and privacy standards.

Qualifications

• Current, unrestricted RN license in North Carolina.
• Minimum of 3 years recent acute care nursing experience.
• Experience with discharge planning or care coordination in a hospital or health plan setting.
• Must be located within 30-60 minute driving distance to Atrium Health Union from home base.
• Experience with Microsoft Office applications and electronic medical records.
• Private workspace for remote documentation.
• Internet service with adequate speeds.
• Dedicated workspace to protect member PHI/HIPAA information.

Provide education on diagnoses, medications, and preventive measures
Complete required documentation accurately and promptly
Maintain infection control, safety, and patient privacy standards

Requirements

• Current, unrestricted Registered Nurse (RN) license in the state of North Carolina
• Must be located within a 30-60 minute driving distance to Atrium Health Union from home base
• Minimum of 3 years recent acute care nursing experience
• Experience with discharge planning or care coordination in a hospital or health plan setting
• Experience with Microsoft Office applications (Word, Outlook, Excel) and electronic medical records
• Valid driver's license, dependable automobile, and proof of personal vehicle liability insurance
• Participation in Tuberculosis (TB) screening and ACHA Level II background clearance, including fingerprinting
• Other credentialing and screening as required by assigned facility
• Private workspace for remote documentation
• Internet service with minimum download speed of 25 Mbps and upload speed of 10 Mbps
• Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
