Job summary

The MetroHealth System in Brooklyn Heights is seeking a Nurse Leader to manage care coordination operations. You will collaborate with leadership to align the continuum of care, perform clinical reviews, and coordinate access and treatment planning to ensure high-quality patient outcomes.

Requirements include a BSN with Ohio RN licensure and at least 5 years of clinical experience, including supervision. EPIC proficiency and strong analytical, communication, and leadership skills are expected.

Qualifications

• BSN and Ohio RN licensure required.

• Strong knowledge of care coordination and discharge planning.
