Qualificações

• Required: Bachelor of Science in Nursing and Ohio RN licensure; 5 years clinical experience.
• Experience in discharge planning and patient throughput across the continuum.
• Strong interpersonal, verbal/written communication, delegation and problem-solving skills.
• Strong analytical, data management and computer skills.

Responsabilidades

• Manage clinical and day-to-day operations of care coordination teams.
• Collaborate with leadership for care coordination across the continuum of care.
• Perform clinical reviews to determine service needs and coordinate patient care within the organization.
• Coordinate access and utilization management, proactive patient management, care facilitation, and treatment planning functions.
