Stanford Health Care in Palo Alto is seeking a capable discharge planning and care coordination professional to join our patient care team. You will work with Case Managers and Social Workers to coordinate resources, communicate with review organizations, and support transitions of care for diverse patient populations.

Formación

• High School Diploma or GED required.
• Three (3) years of progressively responsible and directly related work experience.
• Knowledge of medical terminology and healthcare workflow.

Responsabilidades

• Communicates utilization review, discharge planning and related information to internal and external customers.
• Assists with compiling resource information and developing resource files.
• Gathers clinical, psychosocial and financial information and communicates with appropriate health care team members.
• Participates on patient care teams and contributes to goal setting and process improvements.
• Performs admission, concurrent and retrospective reviews under the direction of the Case Manager.
