Qualifications

• High School Diploma or GED equivalent.
• Three (3) years of progressively responsible and directly related work experience.
• Ability to adjust communications to fit the recipient’s needs and level of understanding.
• Knowledge of medical terminology.
• Knowledge of Windows-based office software, computers and operating systems.

Responsibilities

• Assists with compiling resource information and developing resource files.
• Communicates utilization review, discharge planning and related information to internal and external customers, including nursing staff and review organizations.
• Considers age-specific needs of the patient and communicates with patients, families, staff and diverse communities.
• Documents in the medical record according to guidelines.
• Gathers clinical, psychosocial and financial information and refers patients to professional staff.
• Participates on designated patient care teams and supports goal setting and process improvement.
• Performs admission, concurrent and retrospective reviews under the guidance of the Case Manager.
• Performs clerical activities and data entry related to discharge planning and related areas.
• Reviews daily admission data and checks inpatient insurance benefits and concurrent review requirements.
• Verifies post-hospitalization benefits and coordinates transportation, placement, services and equipment.
