Dignity Health - St. Joseph’s Medical Center in Stockton, CA is seeking a Manager of Care Coordination to lead a multidisciplinary team, oversee daily operations, and drive patient progression to improve outcomes and efficiency.

You will partner with physicians and leadership to remove barriers to care, ensure regulatory compliance, and foster a high-performing culture. Strong analytical, leadership, and mentoring skills are essential in this busy healthcare setting.

Qualifications

• RN license in California (required)
• MSW accepted with instructor exception (No license required for Masters Social Worker)
• Minimum of three years of clinical care coordination or three years of acute nursing leadership experience
• Operational experience in healthcare, staffing, and discharge planning
• CCM or ACM certification within 1 year of hire

Responsabilités

• Provide operational leadership for Care Coordination through supervision, development, coaching, and performance management.
• Coordinate Care Coordination programs and workflows to support patient progression and organizational goals.
• Manage daily department operations including staffing, workload balancing, scheduling, productivity, and resource allocation.
• Supervise front-line Care Coordination staff ensuring accountability for performance and service excellence.
• Ensure timely completion of assessments, discharge planning, utilization management collaboration, documentation, and care transitions per standards.
