Job summary

Wooded Glen Recovery Center is seeking a Case Manager to support the treatment team by coordinating care across multiple providers and ensuring services are delivered per the individual service plan (ISP). You will drive discharge planning and aftercare, maintaining clear communication with referral sources and participating in quality care monitoring.

This onsite position requires a High School Diploma or equivalent, with a Bachelor’s degree in Social Work, Counseling, or a related field

Qualifications

• High School Diploma or equivalent required; Bachelor’s degree preferred.
• 1+ year experience in behavioral health preferred.
• CPR certification and de-escalation training required.
• Onsite facility position with active caseload.

Responsibilities

• Coordinates care across providers to implement the ISP.
• Manages discharge planning and aftercare coordination from admission.
• Maintains comprehensive clinical records and tracks progress toward recovery.
• Collaborates with treatment team, families, and community resources.
• Facilitates referrals and coordinates services with contracted providers.
