Qualifications

• Bachelor's degree in social work or nursing or related field with 1+ year of home and community-based experience.
• RN licensure or MnCHOICES Assessor Certification.

Responsibilities

• Provide care coordination, case management, referrals and follow-up to individuals and families who are members of a vulnerable population and/or high-risk groups.
• Determine eligibility for home and community-based programs and long-term services and support.
• Develop a support plan based on assessed needs and preferences.
• Evaluating and monitoring services identified in the support plan.
• Helping people access needed services.
• Collaborate in delivery of programs and activities for individuals and families that promote health and prevent disease.
