Responsibilities

• Manages patient care coordination across the health care continuum to optimize outcomes.
• Leads process improvements for populations to improve clinical, financial, and satisfaction results.
• Communicates the plan across the continuum of care with physicians and payers.
• Evaluates medical necessity and appropriateness of care to optimize outcomes.
• Collaborates with physicians, payers, and health-care team members.
• Handles denials and educates staff on pre-authorizations and managed care concepts.
