Job summary

BrightCare HomeCare LLC in Baton Rouge, LA is seeking a compassionate Care Manager Assistant to support care managers in coordinating patient care, scheduling, and communications with healthcare providers. You will document assessments, progress notes, and care plans, and collaborate with interdisciplinary teams to tailor plans to each patient.

You will also assist families, address questions, and promote education while helping ensure smooth transitions and high-quality care.

Qualifications

• High school diploma or equivalent; further degree preferred.
• Healthcare or social services experience preferred.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work in a fast-paced environment and meet deadlines.

Responsibilities

• Coordinate patient care with care managers, schedule appointments, and communicate with providers.
• Document patient information, assessments, progress notes, and care plans.
• Collaborate with interdisciplinary teams to create and implement personalized care plans.
• Support patients and families, addressing questions and promoting education.
• Maintain records of patient contacts (calls, visits, emails).
• Participate in quality improvement initiatives.
