BrightCare HomeCare LLC in Baton Rouge, LA is seeking a compassionate Care Manager Assistant to support care managers in coordinating patient care, scheduling, and communications with healthcare providers. Qualifications

Responsibilities

As a Care Manager Assistant, you will play a crucial role in supporting our care managers in coordinating patient care, ensuring seamless transitions, and promoting optimal health outcomes.

Responsibilities:

• Assist Care Managers in coordinating patient care, including scheduling appointments, managing medical records, and communicating with healthcare providers


• Ensure accurate and timely documentation of patient information, including assessments, progress notes, and care plans


• Collaborate with interdisciplinary teams to develop and implement personalized care plans, addressing patients' unique needs and goals


• Provide support and guidance to patients and their families, addressing concerns and questions, and promoting education and awareness


• Maintain accurate records of patient contacts, including phone calls, visits, and emails


• Participate in quality improvement initiatives, identifying areas for improvement and contributing to process enhancements

Requirements:

• High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred


• Prior experience in healthcare, social services, or a related field; minimum of 0 years of experience


• Excellent communication, interpersonal, and organizational skills


• Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and deadlines


• Familiarity with electronic medical records and other healthcare software
