As a Care Coordinator, you will manage care plans, communicate with clients and families, coordinate with nurses and therapists, and monitor progress to deliver high-quality, personalized support. This is a FullTime opportunity with weekly pay and benefits.

• Communicate effectively with clients, families, and healthcare professionals to ensure seamless transitions and optimal care delivery
• Develop and maintain positive relationships with clients, families, and caregivers to promote a sense of trust and respect
• Monitor and evaluate client progress, making adjustments to the care plan as necessary
• Collaborate with our multidisciplinary team, including nurses, therapists, and caregivers, to ensure comprehensive care delivery
• Maintain accurate and detailed records of client interactions, care plans, and client outcomes
• Participate in ongoing training and education to stay current with industry best practices and regulations
