Job summary

Neighborhood Healthcare invites applications for a Social Services Support Specialist. This on-site role, located in Murrieta, CA, supports PACE participants by handling administrative tasks, coordinating community resources, and assisting families in navigating the program.

Under the Social Work Manager, you will ensure safety, confidentiality, and culturally competent service while maintaining HIPAA standards. Schedule is Monday–Friday, 8am–5pm, with local travel to participant homes as needed.

Qualifications

• Bachelor's degree required in Social Work, Psychology, Sociology, Counseling, Human Services, Public Health, Education or related field.
• Associate's degree with 2 years of case management support, outreach, or resource coordination may be considered in lieu of a bachelor’s degree.
• One year of social work experience preferred.
• Current Basic Life Support (BLS) certification required upon hire and maintained as a condition of employment; CPR and First Aid required for all PACE employees.

Responsibilities

• Assist MSW Social Workers by managing documentation and communications, enabling clinical staff to focus on care planning.
• Coordinate referrals to community resources (food, housing, utility, caregiver support).
• Follow up on service referrals and document progress in EMR.
• Prepare documents and notes for care team meetings and IDT meetings.
• Support discharge planning by gathering paperwork and arranging resources.
• Maintain accurate resource lists and participant communication records.
• Participate in team huddles and department meetings; provide input on coordination issues.
