Carease Health - NJ is seeking a compassionate Mental Health Provider to join our Collaborative Care team as a Care Companion/Social Worker, serving residents in skilled nursing facilities in New Jersey.

In this on-site role you will provide emotional support, brief therapeutic interventions, and mental health assessments, coordinating with psychiatric professionals during monthly meetings to ensure holistic care.

Qualifications

• Excellent communication and interpersonal skills for interacting with residents, families and staff.
• Strong documentation and record-keeping abilities for patient notes and care plans.
• Ability to build trusting relationships with older adults in long-term care settings.
• Knowledge of geriatric mental health and crisis management in nursing facilities.

Responsabilités

• Visit designated facilities as scheduled with your supervisor.
• Assess residents to identify emotional, cognitive, and psychosocial needs.
• Provide short-term, solution-focused therapy and supportive interventions.
• Document visit notes and maintain detailed records after each visit.

Job Summary:

Carease Health - NJ is seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team. As a Care Companion/Social Worker, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. This is an ideal role for professionals who want to make a direct impact on the lives of older adults, helping them feel heard, supported, and emotionally connected within their environment.

Key Responsibilities:

• Visit designated facilities in accordance with the schedule set with your supervisor
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
• Document visit notes and maintain detailed records of interactions and interventions after each visit
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications:

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
• Strong communication, documentation, and relationship-building skills
• 1 year of experience in mental health care
