Carease Health is seeking a compassionate Mental Health Provider to join our Collaborative Care team, delivering emotional support, brief therapy, and mental health assessments to residents in skilled nursing facilities.

This role supports older adults, helping them feel heard, supported, and emotionally connected within their environment, with supervision and clinical collaboration provided by our team. A flexible schedule and paid training are offered.

Qualifications

• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Education: Associate Degree
• Experience: 1 year
• Strong communication, documentation, and relationship-building skills.

Responsabilités

• Visit designated facilities in accordance with the schedule with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including monthly meetings with the psychiatric NP.

• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.

Qualifications

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field.
