Carease Health in Miami seeks a compassionate Mental Health Provider to join our Collaborative Care team. You will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

This role offers meaningful work with older adults, ongoing supervision with psychiatric providers, and opportunities for professional growth through monthly clinical meetings and ongoing training.

Qualifications

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.
• Education: Associate Degree
• Experience: 1 year

Responsabilités

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.
