Carease Health is seeking a compassionate Mental Health Provider to join our Collaborative Care team as a Social Work/Care Companion. This part-time role focuses on emotional support, brief therapy, and mental health assessments for residents in skilled nursing facilities.

You will work with an experienced clinical team to deliver high-quality care prioritizing physical and mental well-being in a geriatric population.

Qualifications

• Associates, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.
• Education: Associate Degree
• Experience: 1 year

Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.
