Pay: $19/hr + Mileage reimbursement

The Care Companion is responsible for engaging members (clients) in service by providing direct care to increase socialization and connect to community resources. This position does not have a designated office space within an LSS property.

Responsibilities:

• Conduct program outreach, friendly visits (telephonic and in-person), light household chores, and personal errands such as grocery shopping and transportation.
• Document visits, ensure compliance with regulations, and manage relationships with community stakeholders.
• Provide direct client services, including LSS Healthy Transitions, under supervision.

Qualifications & Requirements:

• Ability to work well with diverse individuals and demonstrate compassion, patience, flexibility, and respect.
• Provide regular in-home/telephonic visits and transportation as needed.
• Dependable transportation and willingness to transport members.
• Computer skills including spreadsheets and word processing.
• Excellent communication and customer service skills.
• Organizational, problem-solving skills, and ability to work independently and in a team.
• Valid driver’s license, clean driving record, and background check.
