Job summary

Midwest Family Services in Zanesville, OH is seeking an Activity Coordinator to plan and deliver engaging activities promoting wellbeing and social interaction for individuals served. You will implement habilitation and vocational activities per each ISP and provide transportation as needed.

The role requires adherence to schedules, documentation of progress, and collaboration with staff in a supportive, person-centered environment. Overtime may be required, with a Monday–Friday first shift.

Qualifications

• Clean driving record a must.
• Will be transporting individuals on a daily basis.

Responsibilities

• Plan, organize and deliver engaging individual and group activities to promote wellbeing and social interaction.
• Provide habilitative and vocational activities as outlined in the Individual Service Plan.
• Maintain documentation including progress notes and medication records as required.
• Interact with individuals and staff professionally and follow policies and ISP guidelines.
• Attend scheduled work hours and participate in training sessions; overtime may be required.
