Job summary

Midwest Family Services in Zanesville, OH is seeking an Activity Coordinator to plan and deliver engaging activities promoting wellbeing and social interaction for individuals served. You will implement habilitation and vocational activities per each ISP and provide transportation as needed.

The role requires adherence to schedules, documentation of progress, and collaboration with staff in a supportive, person-centered environment. Overtime may be required, with a Monday–Friday first shift.

• Clean driving record a must.

Monday-Friday/1st Shift

EXEMPT: No. This is an hourly position. Overtime is paid after 40 hours have been worked in one 7-day workweek. This may be a full-time or part-time position.

SUMMARY: Plan, organizes and delivers engaging individual and group activities that promote wellbeing, social interaction, and mental/physical stimulation, typically in care, community, or day hab settings. Provides habilitative and/or vocational activities and options to individuals served by Midwest Innovations as outlined in their Individual Service Plan. Midwest Innovations serves individuals with mental retardation, and/or mental illness, and/or physical disabilities, and/or developmental disabilities and may display challenging behaviors.

ESSENTIAL DUTIES AND RESPONSIBILITIES: Other duties may be assigned. The word “assist” means the level of assistance required by each specific individual served. Assistance can range from occasional verbal reminders to total assistance by the staff for the individual.

• Provide services and supports as identified in each assigned person’s Individual Service Plan.
• Interact with all individuals and all staff at all times with appropriate voice tone, level, and pitch, language used and gestures and physical movements in compliance with individual rights and staff treatment policies.
• Provide formal and informal training and assistance in activities of daily living; dressing, bathing, eating, cooking, cleaning, etc.
• Provide formal and informal training on all habilitation and vocational activities, helping the individuals to acquire new skills and increase independence.
• Provide training and assistance with self-administration, and administer all ordered medications according to state of Ohio approved guidelines.
• Attend work as scheduled, including training sessions, staff meetings – these are considered scheduled work hours.
• Work overtime hours as scheduled. Although rare, overtime may be mandated for up to 16 hours per week.
• Comply with all duties, policies, procedures, and protocol and other written or verbal directives.
• Provide supervision and support in community settings.
• Complete all skill development and support, progress notes, individual work records, medication administration records and other work-related documents.
• Complete all time sheets; leave slips, accident/incident reports, etc., in a timely manner.
• Ensure a healthy and clean work environment.
• Observe for signs of illness or changes in health and report to supervisor and/or nurse.
• Provide behavior support and crisis intervention as needed and supported by the ISP.
• Plans, organize and conduct calendar meetings
• Coordinate and carry out activities per calendar, per DODD rule
• Provide group activities that support wellbeing and engagement
• Adapt activities to meet physical, cognitive and emotional needs

ADDITIONAL RESPONSIBILITIES:

• Provide training and mentoring for all Day Hab employees to assure all staff is able to complete assignments and answer questions pertaining to job duties.
• Provide training and mentoring to all staff to assure consistent implementation of individual Service Plans.
• Responsible for oversight of facility operations when the Day Hab Specialist 2 is unavailable or as needed.
• Assists management staff with other delegated duties as assigned.
