The Care Coach I empowers caregivers by providing empathetic support, education, and resources to help them care for loved ones with confidence. Using evidence-based methods and digital tools, including phone and proprietary chat technology, the coach addresses both health and social needs. Working with an interdisciplinary team, the Care Coach I primarily supports low-risk caregiver cases across multiple states and Careforth programs. The coach is also responsible for responding to caregiver inquiries, identifying needs, and addressing concerns based on engagement insights.
• Engage, motivate, coach, and educate caregivers via phone and digital technology; empower caregivers to manage their own psychosocial, physical, and environmental health to improve and maintain wellbeing.
• Provide support for multiple caregiver programs, focusing on resource navigation, social determinants of health (SDoH), and care coordination upon request.
• Collaborate with other members of the coaching team to address emerging risks and escalate as appropriate, support care plans, and drive caregiver engagement.
• Identify caregiver challenges and provide resolutions in accordance to internal policies and contractual requirements.
• Document coaching interactions with attention to quality, accuracy, and outcomes, making informed recommendations based on caregiver needs and trends.
• Review and triage alerts as appropriate and conduct manage incident in compliance with applicable state program regulatory and/or contractual requirements.
• Solicit feedback from caregivers to identify opportunities for additional support and process improvement.
• Perform other duties as assigned.
• 2+ years of relevant experience in care/disease management working with complex populations, specifically older adults and/or individuals with disabilities.
• Comprehensive knowledge/experience in managing Medicaid, Medicare, and Managed Care.
• Exceptional active listening and communication skills with a focus on empathy.
• Ability to multitask in a fast-paced environment, with excellent decision-making skills.
• Strong working knowledge of case management systems, community service and delivery systems, as well as Microsoft Office suite.

The pay range for this position is $19.06 - $26.69 per hour. The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.
