Job summary

Avera At Home in Sioux Falls is hiring a compassionate Home Health Aide to support elderly, convalescent, or disabled patients in their homes or facilities.

You will assist with daily living, provide personal care under nurse direction, handle housekeeping and meal prep, and maintain accurate documentation.

Eligible candidates may qualify for a $2500 sign-on bonus. Join a multidisciplinary team dedicated to Moving Health Forward for patients and families.

Qualifications

• Certified Nurse Aide (CNA) license in good standing
• Active driver’s license
• Ability to perform activities of daily living assistance
• Ability to communicate effectively and maintain documentation

Responsibilities

• Provide assistance to patients with activities of daily living.
• Provide personal care for patients as directed and delegated by the nurse.
• Perform housekeeping, laundry, meal preparation, and shopping.
• Assist patient with social activities as prescribed by the plan of care.
• Maintain timely, accurate documentation and report changes to supervisor.
